Registration Information
Registering for Pop Warner Football and Cheerleading requires more paperwork than many sports. We appreciate your cooperation in completing this information for your son and/or daughter. The two main reasons for this level of documentation is to ensure the safety of every participant, and to continue the Pop Warner Football focus on academic achievement. Weight and age of the football players will determine the team assignment, and is strictly enforced by the Pop Warner national organization to ensure a level playing field.

2010 Player Contract/Parental Consent Form (Registration)
This is a basic information about you and your child, and will include you on the PWF list for future communications as the season approaches. It also includes general liability/medical release consent as well as code of conduct requirements.
2010 Physical Fitness and Medical History Form
This is clearance from physician to participate dated after January 1, 2010.
Birth Certificate (Original and Copy)
The birth certificate should be an original or certified copy with a raised seal. In addition, a copy should be submitted so that the original can be returned. With a copy on file, PWF will not have to request the original should your child participate in PWF in the future.
